How to Apply

Orthopaedic Residency

Prerequisites

  • Graduate of an accredited PT program
  • Licensed and in good standing to practice Physical Therapy in the state of California by the start date of the program
  • Reside in the program service area prior to the start of the program

Requirements for Application

  • Copy of Physical Therapy license or date of when licensure exam will be taken.
  • Applicants must show evidence of California Physical Therapy licensure at least six weeks before the program start date.
    • Out-of-state or foreign-trained applicants are encouraged to initiate California licensure as soon as possible.
    • For information regarding California licensure contact the Physical Therapy Board of California.
  • 2 Letters of Recommendation
  • Current CV/resume
  • Program specific essays

Interview Process

Selected applicants will be invited for interviews in May.

Acceptance Notification

Applicants will be notified of acceptance in late May/early June for an October start.

Neurologic Residency

Prerequisites

  • Graduate of an accredited PT program

Application Information

  • You must be licensed and in good standing to practice Physical Therapy in the state of California.
  • Current Kaiser Permanente facilities where the program can support mentoring include San Francisco, East Bay (Oakland, Richmond), Fremont, Union City, San Jose, Santa Clara, Walnut Creek (Shadelands) and Sacramento. Not all facilities support a Fellow-in-Training every year. Contact the program Director in late March for updates.
  • You must be a member of the American Physical Therapy Association (APTA), the California Physical Therapy Association (CPTA), and the Academy of Orthopaedic Physical Therapy (AOPT) and the American Academy of Orthopaedic Manual Physical Therapists (AAOMPT) by the program start date.
  • You must have graduated from an accredited PT residency program or demonstrate an extensive history of continuing education related to orthopaedic practice.
  • Acceptance into the program is competitive. Application requirements will be shared with those interested in late March through May. Interviews of selected applicants occur in June and final selections for program spots are made by mid-July.

Vallejo Fellowship in OMPT & Musculoskeletal Primary Care

Prerequisites

  • Graduate of a CAPTE accredited physical therapy program and eligible for California licensure.
  • Preferred 2 years post-professional musculoskeletal clinical experience.

Include the following in your application packet

  1. A cover letter that addresses the following: (Should not exceed three typed single spaced pages)
    1. What goals do you wish to achieve by participating in a clinical fellowship?
    2. Describe your personal attributes and how these attributes will impact your colleagues in fellowship, clinical faculty, and your profession as a whole.
    3. Professionally speaking, where do you envision yourself in five years?
  2. Curriculum Vitae.
  3. Two letters of recommendation from clinical and/or academic colleagues (one clinical colleague required).
  4. Copy of physical therapy license.
  5. Official academic transcripts from your entry level physical therapy program.
  6. List of continuing education, mentorship, and or research experiences.

Applicants will be notified of acceptance at the beginning of October for a January start.

Kaiser Vallejo PNF Training Program

Prerequisites

  • Graduate or student intern of an approved physical therapy program
  • Must be physically capable of completing job requirements of a therapist working in a neurological rehab setting. Contact us with questions if there are any concerns regarding your ability to physically participate.
  • Foreign-trained applicants:
      • Must have immigration status that allows unrestricted employment in the United States. Kaiser will not be a visa sponsor.
      • Must have graduated from an approved physical therapy program, have proof of professional licensure, and have proof of at least one year of experience in physical therapy after graduation from their curriculum.
      • Must be fluent in English speaking and comprehension (TOEFL – minimum passing score total of 18 in each of the four sections for a total score of 72 or equivalent score in a test of English as a foreign language.)

Upon receipt of the registration form, this office will forward information on completing requisites for PNF Program consideration. To register for the PNF Training Program, submit a completed form to the program coordinator:

Requisites will include the following:

  1. Payment of a refundable registration fee in the amount of $300.00, payable to Kaiser Foundation Hospital is due when registration is submitted. This fee is refunded after completion of the first 30 days of the course. (Fee is waived for Student Physical Therapists)
  2. Photocopy of original diploma or certificate of graduation from physical therapy school – If new graduate or currently completing physical therapy school program, provide letter of verification as to the completion of school requirements signed by either the Clinical Coordinator or Director of the School of Physical Therapy
  3. Photocopy of Physical Therapy license – If new graduate or currently completing physical therapy school program, provide letter of verification as to the completion of school requirements signed by either the Clinical Coordinator or Director of the School of Physical Therapy.
  4. Photocopy of BLS Card of Completion from (American Heart Association)
  5. Current Curriculum Vitae or resumé: Include work experience (job descriptions/titles, dates, employers, employer addresses) and continuing education credits
  6. Two (2) Letters of recommendation from employer, supervisor, professor, etc.
  7. After you are notified of placement in a PNF Course, provide evidence of health insurance for the first month of the assigned course at least 30 days prior to the start of Pre-employment Orientation.

Acceptance Notification

Participants are accepted and notified within 2 weeks of receipt of their completed application/documentation materials.
